I understand that Oaks Park Association, (OPA) will require applicants for employment to take a urinalysis for drug and alcohol screenings and may conduct a criminal background check.
An offer of employment with OPA is conditioned upon the results of my urinalysis for drug and alcohol screens and the background check being satisfactory.
I understand that if I am employed with OPA that the company will require that I submit to a drug or alcohol screen if I am involved in an on-the-job accident, at random, or if OPA has a reasonable suspicion that I am under the influence of drugs or alcohol and I hereby authorize the release of the results of any physical examinations or drug tests required to OPA senior management, and that my refusal to submit to a urinalysis, when requested to do so, may result in termination of my employment. I further understand that OPA may inspect all lockers and any bags, (including purses or briefcases, etc.) or parcels brought into or taken out of Oaks Park.
I understand and agree that if I am offered employment at Oaks Park, my employment will be for no definite term and that either I, or OPA will have the right to terminate the employment relationship at any time, with or without cause, and with or without notice.
